Thank you for choosing to donate to Medicine Hat College. The information gathered in this form will help future generations understand and recognize the purpose and importance of your contribution. Recognition for your contribution will be based on your total tax year giving. In lieu of a charitable receipt, all monthly payroll deduction giving is tallied and will appear on your T4 tax slip.
