Payroll Giving Request Form

Thank you for choosing to donate to Medicine Hat College. The information gathered in this form will help future generations understand and recognize the purpose and importance of your contribution.

Recognition for your contribution will be based on your total tax year giving. In lieu of a charitable receipt, all monthly payroll giving deductions are tallied and will appear on your T4 tax slip.

Aspire: Academic Achievement Fund - Acknowledges learners who excel academically. Grassroots: Access to Education Fund - Connects learners (credit & non-credit) with financial aid. Waldbauer Student Life Enhancement Fund (formerly Pathways) - Provides resources for emergent needs outside of post-secondary fees. Health, Wellness and Athletics Expansion Other (e.g., Student Award, Rattlers Athletics (specify team), Indigenous Student Support Fund, The Beej Project, Pride Fund, Accessibility Access Fund.)
