Pre‑Program Document Requirements

This is a list of all the pre-program documents required to be submitted by the deadline for each program in the YSHCS.

Please consult the following checklist for detailed descriptions of what needs to be submitted, as well as the instructions on how to properly submit each document.

Community Service Programs

Documents required that are NOT within the Learner Orientation Package:

  • Police Information Check with Vulnerable Sector Check

Documents found within the Learner Orientation Handbook:

  • Oath of Confidentiality form
  • General Release form
  • YSHCS Evaluation Release form

Documents required that are NOT within the Learner Orientation Package:

  • Police Information Check with Vulnerable Sector Check
  • Intervention Record Check
  • CPR Certification - Any CPR with first aid component

Documents found within the Learner Orientation Handbook:

  • Oath of Confidentiality form
  • YSHCS Evaluation Release form

No pre-program documentation is currently required.

Health Programs

Documents required that are NOT within the Learner Orientation Package:

  • CPR Certificate - Only Heart & Stroke Foundation Heartsaver AED - Level C CPR or Basic Life Support (BLS) certifications will be accepted
  • Police Information Check with Vulnerable Sector Check
  • Immunizations
  • Intervention Record Check
  • N95 Mask Fitting
  • Valid Class 5 Driver's License
  • Alberta College of Paramedics Registration (if applicable)

  • Heart and Stroke CPR Training form
  • Responsibilities of Students form
  • Satisfactory Standing form
  • Simulation Centre Consent form
  • YSHCS Evaluation Release form
